Hyland Crest
We are putting together a Palliative Team at Hyland Crest. We are looking to compile a list of volunteers that are hospice or palliative care trained who have interest in being added to this contact list. When a resident requires palliative/end of life visitors, the nursing staff may use this tool to reach out to volunteers to see if they are available to partake in palliative visits with the resident at this time.
If you would like to volunteer in this regard, please let me know and I will add you to our contact list. Please let me know the preferred phone number to reach you at for this list.
